Patients registered with us at Thornbrook Surgery are able to access GP services online, including booking nurse appointments, ordering repeat prescriptions and viewing your medical history. 

To register for access to our GP online services, you need to fill in a request from and provide photo ID (e.g. passport, driving licence or bus pass) and proof of address (such as a bank statement or utility bill). Please ensure you tick all the services that you want to use, and read the terms and conditions on the information leaflet provided with the request form.

Once you have signed up, we aim to process your request within 10 working days. We will send you an E-mail with your unique linkage key, the practice "ODS" code and an account ID, together with instructions on how to logon to your chosen service provider. 


Proxy Access

Since April 2020, we are now able to allow a proxy user to oversee someone else's records, e.g. a parent and child. See the flow chart below to check which forms and supporting information we will require. The proxy user (e.g. parent) must have a valid account also as your profile will be attached to their profile.
